I attest that I have read, understand, and agree to the Safety Rules.

I attest that I have read, understand, and agree to the Release of Liability for myself and/or for my child under the age of 18, and that

I attest that I have read, understand, and agree to the Village of Hastings-on-Hudson Code of Ethics for myself and/or for my child under the age of 18.

By signing below, I attest that I have read the Adopt-a-Spot Agreement, its Safety rules and the Maintenance Plan and agree to abide by these guidelines for myself and/or my child under the age of 18 and that I attest that I have read the Release of Liability and the Village of Hastings-on-Hudson Code of Ethics and agree to abide by the provisions therein for myself and/or my child under the age of 18.
